In Network Insurance

I am in network with Aetna, Blue Cross and Moda.

Out of Network Insurance

I bill all other insurance providers as an out of network provider. I will submit the claim and coverage will be determined by your out of network benefit and the specifics of your plan.

Payment

Payment may be made via check, credit card or HSA card via the patient portal.
